Varicocele Treatment

Understanding Its Role in Male Infertility

Nearly 40% of infertility cases are linked to issues with sperm quality or function—commonly known as male factor infertility. One of the conditions that may affect sperm health is a varicocele.

What Is a Varicocele?

A varicocele is an abnormal enlargement and twisting of the veins in the pampiniform plexus within the spermatic cord. This condition can raise the temperature around the testes, which may negatively impact sperm production and function.

Does Treating a Varicocele Improve Fertility?

Varicoceles are usually diagnosed and repaired by urologists through a surgical procedure. While surgery may improve sperm count or motility in some men, research has shown that:

  • Improvements in semen parameters do not always lead to higher pregnancy rates
  • Many couples still require fertility treatments such as IUI, IVF, or ICSI, even after varicocele repair
  • The decision to operate should be based on the couple’s complete fertility profile—not semen analysis alone

In short, better sperm numbers do not always guarantee a higher chance of natural conception.

When Surgery May Not Be Helpful

For men with significantly low sperm parameters—for example:

  • Sperm count below 10 million/ml
  • Motility below 30%

—it is unlikely that surgery alone will improve the semen quality enough to achieve pregnancy naturally. In such cases, couples may benefit more from consulting a fertility specialist for advanced ART options instead of relying on surgery.

A meta-analysis of multiple studies has shown no consistent improvement in pregnancy rates after varicocele surgery, even if sperm counts increase.

Should You Consider Varicocele Surgery?

The effectiveness of varicocele surgery for infertility still remains uncertain. Couples should make an informed decision based on:

  • The female partner’s age
  • Overall fertility health of both partners
  • Semen analysis results
  • Time available for trying naturally

Some couples—especially when the female partner is younger—may still choose to proceed with surgery and try naturally for 6–12 months. The surgery is usually minor, and recovery is quick.

If pregnancy does not occur within that timeframe, options like IUI, IVF, and ICSI are always available at Kiran Infertility Center, Marichela.
